1. Agreement to Receive SMS Messages

By providing your mobile phone number and opting in to receive text messages from Modern Family Medicine, you consent to receive SMS (text) messages sent through an automatic telephone dialing system. Consent is not a condition of purchasing any goods or services from Modern Family Medicine. You may opt in or opt out at any time.

Modern Family Medicine is not responsible for any charges incurred from your mobile carrier. Please contact your mobile carrier for details about your specific plan and any applicable fees.

Our SMS service is available on all major U.S. mobile carriers, including AT&T, T-Mobile, Verizon, Sprint, and others. If you experience issues receiving messages, please contact our office.

6. How to Opt Out

To opt out at any time, text STOP. You can cancel the SMS service at any time by replying STOP, STOPALL, UNSUBSCRIBE, CANCEL, END, or QUIT to any text message you receive from Modern Family Medicine. After you send the STOP message, we will send you a confirmation text that you have been unsubscribed. You will no longer receive SMS messages from us unless you re-enroll by contacting our office or opting in through our website or forms.
